Delegated Medical Services

In accordance with Board Rule 1.17 and HB 25-1024, Alchemy Face Bar and its Medical Director, Michael R. Iannotti, MD, are providing you with medical-grade aesthetic services that may be delegated by a licensed physician to trained and certified licensed or unlicensed individuals. Please review the following information:Medical Oversight: Michael R. Iannotti, MD, serves as our Medical Director. He is board-certified in Family Medicine and extensively trained in medical-grade aesthetic consultations and procedures.

1. Delegation of Services: Dr. Iannotti delegates his authority to perform certain aesthetic procedures to licensed and unlicensed contractors and employees of this medical spa. All delegatees have received appropriate training and certification to safely provide these services.
2. Scope of Practice: Any delegatee performing services outside of their standard scope of practice does so under Dr. Iannotti’s delegation, having received advanced training and certification from recognized third-party agencies.
3. Availability: Dr. Iannotti is available for consultation before or after any procedure, as well as for any concerns, complications, or follow-up care.

7. Access to Information: A copy of this information can also be found on our website.
8. Recordkeeping: This informed consent will be posted in the spa’s reception area and saved in your medical chart for a minimum of seven (7) years, in compliance with state law. After this period, charts may be lawfully destroyed.
